You can view a blank version of any survey you created by going to the Form Builder widget on the lefthand navigation panel:

Once you click that widget, you will be taken to the page where all the forms you have created are listed:

Click on the down arrow button to the right of the survey you wish to see to download a CSV file that can be viewed in Excel:
